Comments:
An unannounced monitoring inspection was conducted on 6/4/19. At the time of entrance 24 participants were present with 7 center staff including the Director. This exceeds the required ratio of one staff per six participants. The sample size consisted of six participant records and three staff records. Participant interviews were not conduct as participants do not speak English and no interpreter was provided. Participant and staff records and other documentation reviewed. Criminal Background Checks of all staff hired since previous inspection were reviewed. Participants were observed eating and engaging in activities. No participants received medications during the inspection. Violation notice issued and exit interview held. Standard #: 22VAC40-60-90-A
Description: Based on documentation review, the facility failed to comply with the terms of their license: Per Building Official all participants must be ambulatory. As evidenced by: One of six participant records reviewed had documentation of a non-ambulatory participant. Participant # 4's admitting History and Physical (H&P) dated 4/22/19 had the doctor assess the participant, "By reason of physical or mental impairment is not capable of self-preservation with the assistance of another person." This makes the participant non-ambulatory.

Plan of Correction: The Administrator will have the participant re-assessed. If the participant is non-ambulatory the Administrator will assist family in finding an appropriate placement and will immediately discharge.
