Comments:
The LI for Bedford Adult Day Care Center conducted an unannounced monitoring visit at the facility on 6/13/19 from 12:30pm until 2pm and noted 41 participants and 10 staff. Participant and staff records as well as other forms of facility documentation were reviewed. A tour of the physical plant was conducted and the afternoon activity was observed. Violations:
Standard #: 22VAC40-60-510-G
Description: Based on physical plant observations, the facility failed to ensure that hot water from taps available to participants was maintained within a range of 105'F to 120'F. EVIDENCE: 1. The hot water from the taps in bathrooms 1 and 2 were noted to be 100'F at the time of inspection.

Plan of Correction: The Director will have maintenance adjust hot water to reach the correct temperatures and monitor regularly.

Standard #: 22VAC40-60-600-B
Description: Based on participant record reviews, the facility failed to ensure the physical examinations were completed prior to admission. EVIDENCE: 1. The physical examination in the record for participant 1 is incomplete as it lacks documentation from the physician as to whether the participant is or is not capable of administering his own medications without assistance or a statement that the individual is or is not physically and mentally capable of self-preservation by being able to respond in an emergency.

Plan of Correction: The Director will have the physical examination updated to include all required information and will review all examinations prior to participant admission for completion.
