Comments:

An unannounced monitoring inspection was conducted on 7-2-19 (af 09:45 am/dep 12:20 pm). The facility census upon arrival was 18. There were three staff and the director present. A tour of the facility was conducted, participant and staff records were reviewed, posted documents were observed and medication pass observation was conducted with the director. The participants and staff were engaged in various activities (exercise, memory game, morning news, etc). The first aid kit was also checked. The acknowledgement form was signed following the exit interview. Comment: The licensing inspector spoke with the director regarding the use of gait belts in the facility and documentation of staff receiving gait belt training.
