Comments:

Two licensing inspectors conducted an unannounced monitoring inspection at Pulaski Adult Day Service & Fall Prevention Center, Inc. on 07/18/2019. The inspection started at 10:40 am and concluded at 11:53 am. Participant files were reviewed, staff filed were reviewed, and a tour of the building and grounds was conducted. The first aid kit was observed and reviewed. The facility was found to have 13 participants on the day of this inspection. Required postings were observed. Previous violations were reviewed to determine correction and compliance with standards. An exit meeting was held with the administrator on 07/18/2019 and the findings were reviewed.
