An unannounced monitoring inspection was conducted on 7/22/19. At the time of entrance 17 participants were present with seven center staff including the Director. This exceeds the required ratio of one staff per six participants. The sample size consisted of six participant records and three staff records. There have been no new staff hired since previous inspection. Spoke with participants. Participants were observed eating and engaging in activities including morning exercise, and a biography program. Medications administration was observed.
