Comments:
Two licensing inspectors conducted an unannounced mandated monitoring inspection at Generations at Haysi on 08/22/2019. This inspection started at 10:45 am and concluded at 11:40 am. The center reported 14 participants enrolled with 6 participants attending on the day of the inspection with 4 staff providing care and services. Participant and staff records were reviewed; a tour of the center was conducted; activities were observed throughout the morning and the noon meal was observed. An exit meeting was held with the center director on 08/22/2019. An opportunity was given during the exit meeting to find items not available during the inspection. The findings of the inspection were reviewed. As a result of this inspection 2 violations are being cited. Please complete the "description of action to be taken" and "date to be corrected" for each violation cited on the violation notice, and then return the signed and dated copy to the licensing office within 10 calendar days (09/08/2019) of receipt. Standard #: 22VAC40-60-450-D
Description: Based on observations made during the morning tour, the facility failed to keep cleaning products and harmful materials in a locked place when not in use.

EVIDENCE:
1. In the common sink area of the center, there was a container of 75 Clorox Disinfecting Wipes sitting beside of the sink. The label on the Clorox Disinfecting Wipes container stated ?keep out of reach of children?

Plan of Correction: Educated all staff on regulations of cleaning supplies; Cleaning products are harmful materials will be kept in a locked place when not in use. [sic].

Standard #: 22VAC40-60-860-B
Description: Based on observations made during lunch, the facility failed to indicate substitutions made on the menu.

EVIDENCE:
1. The lunch menu for 08/22/2019 showed grilled chicken, tossed salad with ranch, baked potato, blueberry cobbler, and a wheat roll would be served.
2. One licensing inspector observed lunch. The participants had grilled chicken, peas, baked potato, and a piece of wheat sandwich bread.

Plan of Correction: Educated staff, especially those preparing and serving food, that any substitutions need to be reported to the supervisor and also indicate substitutions written on menu for participants to see. [sic]
