Comments:

An unannounced monitoring inspection was conducted on 8/27/19. At the time of entrance nine participants were present with five center staff including the Director. This exceeds the required ratio of one staff per six participants. The sample size consisted of four participant records and three staff records including the criminal background checks on all new staff hired since previous inspection. Spoke with participants. Participants were observed eating and engaging in activities including morning snack, current events, and family fun .No medications were given. No violations were issued and exit interview held.
