TA provided regarding annual training hours and plan of care documentation.

An unannounced monitoring inspection was conducted at the center on 10/3/2019. The center reported 48 participants in care, 31 were on site on the day of the inspection from approximately 10:30 a.m. to 1:15 p.m. Licensing staff reviewed all required program and file documentation for compliance along with 4 participant and three staff files. An inspection of the center found that the center was clean and in goood repair, participants were observed eating a lunch meal and participating in individual activities. No violations were found during the inspection.
