Comments:

An unannounced renewal inspection was conducted on 11/14/2019. At the time of entrance 15 participants were present with six center staff including the Director. This exceeds the required ratio of one staff per six participants. The sample size consisted of four participant records and three staff records including the criminal background checks on all new staff hired since previous inspection. Spoke with participants. Participants were observed having morning snack and engaging in activities including A to Z Trivia with the letter L, Welcome/am news, exercise, and current events . Medications were reviewed.
