If you have not already done so you must submit a completed Renewal Application prior to the expiration of your current license. The application can be obtained from our web site.
Please Note: While the Fire Inspection remains current it will be expired prior to the date of expiration on your license. Please email or fax a copy of the new Fire Inspection once it is obtained so that we can proceed with processing your new license.

An unannounced renewal inspection was conducted on 11/18/19. At the time of entrance 28 participants were present with 10 center staff. This exceeds the required ratio of one staff per six participants. The sample size consisted of five participant records and nine staff records including the criminal background checks on all new staff hired since previous inspection. Spoke with participants. Participants were observed having morning snack and engaging in activities including morning exercise and current events. Medications administration was observed and and no violations were issued. Exit interview held.
