Comments:

The LI for Bedford Adult Day Care Center conducted an unannounced renewal study at the facility on 12-2-19 from 10am until 1pm and noted 40 participants and 10 staff in the facility. Participants and staff records as well as other forms of facility documentation were reviewed and interviews were conducted with staff. A tour of the physical plant was conducted and the morning hangman word activity and the mid day meals were observed. If you have any questions please feel free to contact your LI at 540-309-2968.

Description:	Based on observations of the facility physical plant, the facility failed to maintain the interior of the building. EVIDENCE: 1. The plaster at the bottom of the column near the front door was noted to be cracking and had missing pieces.
Plan of Correction:	The Director will contact maintenance to have the column repaired.
