Comments:

An unannounced mandated monitoring inspection was conducted on January 30, 2020 from 9:30 am until 2:51 pm. There were 9 participants at the start of the inspection and 12 participants by lunch time. During the inspection a tour of the physical plant was conducted, a medication administration, and activities were observed. Participant and staff records were reviewed. Lunch was served as posted on the menu. Briefly reviewed changes and additions to new regulations regarding participant rights, staff training requirements, participant agreement, and categories of scheduled activities. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice. The plan of correction must indicate how the violation will be or has been corrected. The plan of correction should include: 1. Step(s) to correct the non-compliance with the standard 2. Methods to prevent re-occurrence 3. Person(s) responsible for implementing and/or monitoring any preventive action(s).

Standard #:	22VAC40-61-160-A-1
Description:	Based on record review and interview, the center failed to ensure each direct care staff member maintained current certification in first aid. Evidence: 1. Staff #3 did not have a current certification in first aid. Staff #3's first aid certification expired on 12-14-2019. 2. Staff #1 acknowledged staff #3's first aid certification had expired.
Plan of Correction:	Staff #3 will take a First Aid class on 3/12/2020. This was the first class available that staff #1 could find for her.
