Comments:

The licensing inspector for Pulaski Adult Day Service and Fall Prevention Center conducted an unannounced mandated licensing renewal inspection on 03/03/2020. The inspection started at 10:15 am and concluded at 12:15 pm. The center is licensed to provide adult day services to 30 adults and five participants were found to be in care during the inspection. A tour of the center was conducted. Participant and staff files were reviewed. Activities and the noon meal was observed. Required postings were observed to be in place. As a result of this inspection, no violations are being cited. An exit meeting was conducted with the director on 03/03/2020.
