Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 5/26/2020 and concluded on 5/28/2020. The director was contacted to initiate the inspection. There were 0 participants present and 0 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed three participant?s records and three staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. Violations:

Standard #:	22VAC40-61-180-D
Description:	Center failed to ensure that all employee records included an original Virginia State Police criminal record report. Evidence: Staff #1, #2 and #3 did not have Virginia State Police Background Checks in the staff record.
Plan of Correction:	A third party state criminal background check was completed on each staff member. A Virginia State Police background check will be completed for all staff.
