Comments:

A renewal inspection was initiated on 6/3/2020 and concluded on 6/4/2020. The Executive Director was contacted by telephone to initiate the inspection. The Executive Director reported that the current census is zero (0). Facility temporarily closed on March 23, 2020 due to the COVID 19 pandemic and is scheduled to re-open on 6/8/2020. The inspector emailed the Executive Director a list of items required to complete the inspection. The inspector reviewed two (2) participant records, two (2) staff records, activities calendar, staff schedules, menus, fire and health inspection reports, emergency evacuation drills, etc. submitted by the facility to ensure documentation was complete. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
