Comments:

A monitoring inspection was initiated on June 1, 2020 and concluded on June 4, 2020. The Administrator was contacted by telephone to initiate the inspection. There were 9 participants present and five staff. The inspector emailed the Administrator a list of items required to complete the inspection. The Inspector reviewed three participants records and three staff records submitted by the facility to ensure documentation was complete. The information gathered during the inspection determined no violation with applicable standards or law. No violations were issued. The Licensing Inspector has reviewed with the Administrator COVID-19 guidance posted on the ADCC webpage.
