Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 6/18/2020 and concluded on 6/23/2020. The director or in-charge person was contacted by telephone to initiate the inspection. There were 29 participants present and 9 staff. The inspector emailed the director/person in charge a list of items required to complete the inspection. The Inspector reviewed 3 participant?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the director/person in charge COVID-19 guidance posted on the ADCC webpage.

Standard #: 22VAC40-61-230-D
Description: Based on a review of participant records, the facility failed to ensure that all identified needs were included on participant plan of cares (POC).

EVIDENCE:

1. The assessment dated 6/1/2020 in the record for participant 1 has documentation that the participant is a risk for falls. The POC dated 1/13/20 does not address this identified need.

2. The assessment dated 6/1/2020 in the record for participant 2 has documentation that the participant is disoriented to place and time. The POC dated 5/22/2020 does not address this identified need.

Plan of Correction: The Director of Nursing will correct these participants POC's to include all identified needs and will conduct routine audit of all participant POC's to ensure all identified needs are addressed.
