Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 7/8/2020 and concluded on 8/4/2020. The director or in-charge person was contacted by telephone to initiate the inspection. There were seven (7) participants present and four (4) staff. The inspector emailed the director/person in charge a list of items required to complete the inspection. The Inspector reviewed two (2) participant?s records and two (2) staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the director/person in charge COVID-19 guidance posted on the ADCC webpage.

Standard #:	22VAC40-61-230-A
Description:	Based on participant record review, the facility failed to address assessed needs on a plan of care (POC). EVIDENCE: 1. The participant assessment for participant 2 dated 3/2/2020 shows this person needs moderate assistance with bathing and mouth care, requires total assistance with dressing, transfering, and ambulation. These needs are not addressed on the POC dated 3/2/2020. The need of supervision with bathroom/hygiene needs are addressed on the POC, but the services provided are not specific.
Plan of Correction:	Update POC to reflect the services that will be provided and have singed by caregiver.
Standard #:	22VAC40-61-300-C
Description:	Based on participant record review, the facility failed to have a verbal medication order reviewed and signed by the physician or prescriber within ten (10) working days. EVIDENCE: 1. Participant 2 has a verbal order for prednisone 10 mg dated 7/1/2020. As of 8/4/2020, it has not been signed by the physician or prescriber.
Plan of Correction:	Obtain an order signed by MD for current prednisone order and obtain & secure a signed order for any future orders within 10 working days.
