Licensing Inspector (LI) conducted an announced initial inspection on 9/10/2020. LI walked the physical plant and verified room measurements. The Center's policies and procedures were reviewed off site prior to the inspection. The Fire Marshall, Health Department and Building Official Inspections have been completed and submitted. The cited violations were reviewed with the owner during the exit interview held immediately following the inspection. Standard #:	22VAC40-61-300-B
Description:	Center failed to ensure that at least one pharmacy reference book, drug guide, or medication handbook is readily accessible as reference materials that is no more than two years old.
Plan of Correction:	Owner will purchase reference materials prior to opening.
Standard #:	22VAC40-61-360-E
Description:	Center failed to ensure that a current copy of a diet manual containing acceptable practices and standards for nutrition shall be available to staff responsible for food preparation and meal planning.
Plan of Correction:	Owner will purchase diet manual prior to opening.
Standard #:	22VAC40-61-460-E
Description:	Center failed to ensure that sturdy grab bars or safety frames shall be installed beside all toilets used by participants. Evidence: One toilet in the men's restroom and two toilets in the women's restroom did not have sturdy grab bars or safety frames installed.
Plan of Correction:	Owner will have sturdy grab bars or safety frames installed prior to opening.

Description:	Center failed to ensure that there shall be an adequate supply of disposable gloves in each restroom at all times.
Plan of Correction:	Owner will install glove box holders on the wall of each restroom and shower room to ensure disposable gloves are available in all the restrooms at all times.
