Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A mandated monitoring inspection was initiated on 11/04/2020 and concluded on 11/09/2020. The Center Director was contacted by email to initiate the inspection. The Center Director reported that the current census was 5. The inspector emailed the Center Director a list of items required to complete the inspection. The inspector reviewed two resident records, two staff records, medication administration records, local fire and health inspection and other documentation submitted by the facility to ensure documentation was complete. No new staff have been hired since the most recent inspection. Please note that the center suspended operations beginning March 16, 2020 and reopened on September 28, 2020 with health/safety restrictions in place. All residents were reassessed prior to attending. New policies and procedures were reviewed and approved. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
