Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 1/15/2020 and concluded on 1/19/2020. The director or in-charge person was contacted by telephone to initiate the inspection. There were 6 participants present and 5 staff. The inspector emailed the director/person in charge a list of items required to complete the inspection. The Inspector reviewed 2 participant?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the director/person in charge COVID-19 guidance posted on the ADCC webpage.

Standard #:	22VAC40-61-230-A
Description:	Based on record review and discussion, the facility failed to develop a preliminary multidisciplinary plan of care based upon the assessment for each participant prior to or on the day of admission. Evidence: 1. Participant #2?s date of admission was 9/28/2020; however the plan of care is dated 10/14/2020. 2. Staff #1 acknowledged that he plans of care for Participant #1 and Participants #2 were not completed prior to or on the date of admission.
Plan of Correction:	A copy of all multidisciplinary plans of care for currently enrolled participants will be reviewed with the caregiver by the nurse at each location and will be signed by the caregiver. Going forward all multidisciplinary plans of care based upon the assessment for each NEW participant will be developed prior to or on the date of admission.
