Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 2/9/21 and concluded on 2/10/21. The director or in-charge person was contacted by telephone to initiate the inspection. There were 26 participants present and 14 staff. The inspector emailed the director/person in charge a list of items required to complete the inspection. The Inspector reviewed 3 participant?s records and 3 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the director/person in charge COVID-19 guidance posted on the ADCC webpage.

Violations:
Standard #: 22VAC40-61-260-B
Description: Based on record review and discussion, the facility failed to ensure the participant obtain a tuberculosis (TB) screening by a qualified licensed practitioner no earlier than 30 days before admission.
Evidence:
1. Participant #3?s date of admission is 1/1/21. The participant?s record did not contain a TB screening.

2. Staff # 1 acknowledged Participant #3 did not have a TB screening completed as required, and therefore could not provide a copy.

Plan of Correction: The Director of Nursing educated the staff on the importance of completing the TB screening during the initial assessment for enrollment in the program.
The clinic RN supervisor will complete an audit each month there are new enrollments to ensure the TB screenings were completed.
