Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 2/22/21 and concluded on 2/23/21. The director or in-charge person was contacted by telephone to initiate the inspection. There were 2 participants and 4 staff. The inspector emailed the director/person in charge a list of items required to complete the inspection. The Inspector reviewed 2 participant?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance (s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the director/person in charge COVID-19 guidance posted on the ADCC webpage.

Violations:
Standard #: 22VAC40-61-260-B
Description: Based on record review and discussion, the facility failed to ensure the physical examination report on file at the center included all necessary documentation.
Evidence:
1. Participant #1?s physical examination report dated 1/13/21 documented an allergy of Flagyl; without a description of the reaction to the allergy; and did not document the participant?s ambulatory status.

2. Participant #2?s physical examination report dated 11/3/2020 documented allergy of Benazepril, Morphine, and Biphosphates; without a description of the reactions to the allergies; and did not document the participant?s ambulatory nor medication administration statuses.

Plan of Correction: PCP will be asked to add allergies if any and to document the reaction from either medication or food. Ambulatory status will be added on physical forms for all participants.
