Please ensure that prior to offering day care services that all staff have updated TB Risk Assessments completed.

Comments:

Due to the current pandemic the Adult Day Care Center did not offer services during the conditional licensure period thus inspections were not completed. Based on the renewal application and a meeting with center owner it was determined the the center continues to have the capability to operate including staff, training and facilities.
