Comments:

A renewal inspection was initiated on October 28, 2021 and concluded on November 1, 2021. The Center Director was contacted by email to initiate the inspection. The Center Director reported that the current census was eight. The inspector emailed the Center Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed two participants records, two staff records, menus, activities calendar and staff schedules. submitted by the facility to ensure documentation was complete. .The facility does not have anyone they administer medication to at this time, but there is one as need medication that has never been administered to the participant. An exit interview was conducted with the Center Director on November 1, 2021, where findings were reviewed and an opportunity was given for questions.
