Comments:

Two licensing inspectors conducted a one day mandated monitoring inspection at Pulaski Adult Day Service & Fall Prevention Center on 02/14/2022. The inspection began at 10:40am and concluded at 11:55am. The licensing inspectors observed the center, reviewed participant records, reviewed the scheduled, menu, and activity calendar. An exit meeting was held with the administrator of the center. One violation is being cited as a result of this monitoring inspection. Standard #:	22VAC40-61-180-E-2
Description:	Based on observations made during staff record review, the center failed to have a subsequent tuberculosis evaluation completed in a timely manner for one employee. EVIDENCE: 1. Staff #2's last tuberculosis was dated 01/20/2021. The next evaluation is already scheduled for later this month.
Plan of Correction:	The employee has a TB screening scheduled and the director will monitor TB screenings monthly. [sic]
