Comments:

An initial announced onsite inspection was was conducted on 3/16/22 (1:30pm to 3:00pm) by two Licensing Inspectors. Measurements were taken for the requested capacity for licensure. The inspectors inspected the facility and mandatory documents for compliance. Information gathered during the inspection determined non-compliances with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Standard #:	22VAC40-61-460-E
Description:	Based on observation made during an inspection of the facility, the facility failed to ensure sturdy grab bars or safety frames were installed beside the participant toilet seat. Evidence: 1. On 3/ 16/22 during a physical plant inspection of the facility, the participant toilet did not contain grab bars or safety frames. 2. Staff #1 acknowledged there were no grab bars or safety frame near the participant toilet.
Plan of Correction:	Grab bar has been installed at toilet area for participants.
Standard #:	22VAC40-61-460-H
Description:	Based on observation and facility inspection, the center failed to ensure hot water at taps available to participants are maintained within a temperature range of 105 degrees Fahrenheit and 120 degrees Fahrenheit. Evidence: 1. On 3/16/22 during a tour of the center, the hot water temperature was checked in the bathroom for participant use. The temperature only reached 98 degrees Fahrenheit. 2. Staff #1 acknowledged the hot water was not within a temperature range of 105 degrees Fahrenheit to 120 degrees Fahrenheit.
Plan of Correction:	Hot water temperature will be adjusted so that the facility water will reach desired temperature by adjusting the main valve.

Description:	Based on observation during the physical inspection of the facility, the center failed to ensure if bathing facilities are provided there are handrails in shower stalls and bathtubs. Evidence: 1. On 3/16/22, during a tour of the center with Staff #1 a bathtub was observed in the participant bathroom. There were no handrails near the bathtub. 2. Staff #1 acknowledged there were no handrails near the bathtub.
Plan of Correction:	Grab bars have been installed in front of shower/bathtub and handrails have been added inside of bathtub/shower area.
