Comments:

An unannounced mandated monitoring inspection was conducted on 4/5/2022. At the time of entrance 10 participants were present with 5 center staff; this far exceeds the requirement of one staff per six participants. The sample size consisted of two participant records and two staff records. Participant and staff records and other documentation reviewed. No new staff have been hired since previous inspection conducted on 5/6/2021. Participants were observed engaging in activities including morning detective, exercise and a virtual music activity with another center. No participants received medications during the inspection.
