Comments:
An unannounced monitoring inspection was conducted on 3-22-22 from 08:30 am to 11:35 a.m. There were four participants present and three staff when inspector arrived on site. Staff and participants were engaged in morning snack and an activity. Two staff and two participant records were reviewed with staff. An exit interview was conducted with the director. The acknowledgement document was emailed to the director on 3-24-22.
Comment: The director was informed of the new application process, online and electronic payment. The director was also reminded of the office's new address for mailing items.
Please complete the 'Plan of Correction' and 'Date to be Corrected' for each violation cited on the violation notice and return it to me within 10 calendar days from today. You need to be specific with how the deficiencies either have been or will be corrected to bring you into compliance with the Standards. Your plan of correction must contain the following three points: 1. Steps to correct the noncompliance with the standard(s) 2. Measures to prevent the noncompliance from occurring again 3. Person(s) responsible for implementing each step and/or monitoring any preventive measure(s) Please provide your responses in a Word Document, if possible. The plan of correction is due by 4-10-22. Violations:
Standard #: 22VAC40-61-550-A
Description: Based on observation and staff interviewed, the facility failed to ensure the first aid kit in the center included all required items per the regulation.

Evidence:
1. On 3-23-22 the first aid kit was checked with staff #2 and did not include the following required items: (a) antibacterial ointment and (b) ice pack or ice bag.
2. Staff #1 and #2 acknowledged the first aid kit did not include the aforementioned items when checked for compliance.

Plan of Correction: 1) Items were immediately placed back in the kits (the ice pack and antibacterial ointment).
2) To prevent this from happening again, the kits will be checked monthly to ensure all items are present and not expired.
3) Director will be the person responsible for checking kits.
