Comments:

An unannounced, renewal inspection was initiated on 04-22-2022 by Licensing Inspector from 12:12 p.m. until 1:30 p.m. Upon arrival, there were 3 staff members present. The center reopened 04-18-2022 and does not have any participants enrolled at the time of the inspection. Following a tour of the center, three staff records were reviewed. The menu, activity calendar, health and fire inspection, building first aid kit, and water temperature were reviewed as well. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Standard #:	22VAC40-61-180-E-2
Description:	Based on record review, the center failed to ensure all staff and volunteers have an annual tuberculosis risk assessment completed. Evidence: 1. Staff #2 had a chest x-ray on 12/20/2019; however, it is not consistent with the TB risk assessment as published by the Virginia Department of Health or indicate Staff #2 is free from TB in a communicable form.
Plan of Correction:	Staff #2 made an appointment at Urgent Care 4/27/2022. Staff #2 received an updated TB risk assessment that indicates Staff #2 is free from TB in a communicable form.
