Description:	Based on participant record review and interview, the facility failed to address some known needs on the Plan of Care (POC). EVIDENCE: 1. The participant assessment for participant 1 dated 3/22/2022 shows the following needs, which are not addressed on the POC dated 1/6/2022: standby assistance needed with walking and climbing stairs; standby assistance needed with bathing and transferring; assistance with dressing and toileting, and incontinence; needs time to communicate. Interview with staff 1 conforms that the participant assessment for participant 1 was correct. 2. The participant assessment for participant 3 dated 3/27/2022 shows the following needs, which are not addressed on the POC dated 5/11/2022: supervision with climbing stairs; assistance needed with bathing and dressing. Interview with staff 1 conforms that the participant assessment for participant 3 was correct.
