Type of inspection: Monitoring
An unannounced mandated monitoring inspection conducted on-site on 8-9-22 (Ar. 09:20 a.m./dep 14:10 p.m.) The facility census was 15 and there were four staff not including the Director present. The morning snack was being served and morning reading activity conducted, exercise program also conducted; medication pass observation conducted, water temperature and emergency preparedness information posted conducted. A review of staff and participants records were reviewed. The new inspection process explained to the Director, including final exit meeting requirement.
The Acknowledgement of Inspection form was sent via email to the Director for review and signature and date.

The final exit meeting was conducted on 8-11-22.

The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.

Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.

Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.

The department's inspection findings are subject to public disclosure.

Violations:
Standard #: 22VAC40-61-60-B
Description: Based on observation, document reviewed and staff interviewed, the facility failed to ensure the current license was posted in the center in a place conspicuous to the participants and the public.

Evidence:
1. On 8-9-22, the license posted in the facility foyer area was dated September 20, 2019 through September 19, 2021.
2. On 8-9-22, staff #l acknowledged the current license was not posted in the facility.

Plan of Correction: Current license will remain on wall in frame at the entrance of our center at all times.

Standard #: 22VAC40-61-340-B
Description: Based on document reviewed and staff interviewed, the facility failed to ensure when any portion of an adult day care center is subject to inspection by the Virginia Department of Health, the center shall be in compliance with those regulations, as evidenced by a subsequent annual report from the Virginia Department of Health.

Evidence:
1. On 8-9-22, the health inspection document presented for review was dated 8-26-19.
2.On 8-9-22, staff #1 and #4 acknowledged the facility did not have a current health inspection, the last health inspection document for the ADCC was dated 8-26-19

Plan of Correction: Food Establishment Inspection was held on 8-12-2022 and is now posted at the entrance of our center and in the kitchen along with the current permit. Moving forward, our Dining Services Director and the Director of Adult Day Center will ensure that if the inspection is coming due, and the inspector has not arrived within once month prior to its expiration, that calls and emails will be made to the current Environmental Health Specialist.
