Description:	Based on participant record review, the facility failed to address some needs and services provided by the center on a plan of care (POC). EVIDENCE: 1. The participant assessment (PA) for participant 1 dated 5/25/2022 showed this person needs a walker when walking (mechanical assistance), and supervision with stairs and bathing. These are not addressed on the POC dated 5-23-2022. 2. The PA for participant 3 dated 3-22-2022 shows this person needs assistance with walking, stairs, bathing, transferring, eating/feeding and these are not addressed on the POC dated 3/12/2022.

Description:	Based on observation and resident record review, the facility failed to implement its medication management plan. EVIDENCE: 1. Regarding methods to secure and maintain supplies of each participant?s medications, the facility?s plan states, ?All Meds checked daily and reported to family for refills by the way daily report sheet.? Participant 4 has instructions to take Tylenol 650 mg, both scheduled and as needed, and this medication is not in the facility. Tylenol 500 mg is on hand, instead.
