Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 4/15/2021 and concluded on 4/19/2021. The administrator was contacted by telephone to initiate the inspection. The administrator reported that the current census was 55. The inspector emailed the administrator a list of items required to complete the inspection. The inspector reviewed (4) four resident records, (4) four staff records, and other required documentation were submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-73-680-E
Description: Based on a review of the residents medication administration record documentation procedures ordered by a physician were not provided according to his instruction.
Evidence:
An order for Accu checks to be obtained at 9pm x 5 days for resident #2 were not followed. Accu checks on 3-4, 3-5, 3-8, 3-9- 2021 were documented "not in stock"; 3-6-2021 " do not have equipment to do check"; 3-7-2021 "not available-back order".

Plan of Correction: Each RMA on that floor has been instructed on the proper procedure for procuring equipment to perform accu checks either from the pharmacy or another vendor. All RMA's say they understand the process and abide by it vs writing "not in stock", "not available", "do not have equipment to do check".
Director of Health and Wellness will monitor accu checks monthly.
