Comments:

An unannounced monitoring inspection was conducted on 5/28/19 by 1 licensing inspector from approximately 10:45 a.m. - 1:15 p.m. The current resident census reported is 60. During the inspection, a tour of the facility was conducted, a sample of 8 resident and 4 staff records were reviewed, facility documentation was reviewed, meal and activity calendars were reviewed, lunch meal was observed, first aid kit supplies were reviewed, medication pass was observed, medication administration records and physician orders were reviewed, interview of staff and residents were conducted and no issues or concerns were reported. An exit meeting was held. There was a violation cited during this inspection. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return to the Inspector within 10 days. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description:	Based on review of resident records and facility documentation, the facility failed to ensure that all direct care staff maintain current certification in first aid. Evidence: The last first aid certification available for review at the facility for staff #1 was issued on 1/20/17 (certification expired in January 2019).
Plan of Correction:	First Aid and CPR class was completed by staff #1 on 6/7/19. Going further, Administrator will ensure that all employee certifications are current.
