Date of inspection and time the licensing inspector was on-site at the facility on the day of the inspection: 10/18/22, 12:30-4:00 pm
The Acknowledgement of Inspection form was signed and left at the facility for the date of the inspection.
A self-reported incident was received by VDSS Division of Licensing on 10/18/22 regarding allegations in the areas of Additional Requirements For Facilities That Care For Adults with Serious Cognitive Impairments.
An exit meeting was conducted to review the inspection findings.
The evidence gathered during the investigation supported non-compliance with standard(s) or law, and a violation was issued in the areas of Staffing and Supervision and Additional Requirements For Facilities That Care For Adults With Serious Cognitive Impairments. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.
If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.
Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.
Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.
Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.
The department's inspection findings are subject to public disclosure.
Please Note: A copy of the findings of the most recent inspection are required to be posted on the premises of the facility.

Standard #: 22VAC40-73-1130-A Description: Based on a review of the staff schedule and an interview with the licensee, the facility failed to maintain staffing requirements for the safe, secure environment.
Evidence: The staff schedule document and the licensee confirmed that there was one staff working on the safe, secure environment on the 7 pm to 7 am shift on 10/16/2022 at the time of the incident.
The staff schedule from 10/3 to 10/16 also document only one staff working on the safe, secure unit on the 7pm to 7am shift on 10/5, 10/6, 10/12, 10/15.
The license reported that there are 19 residents in care in the safe, secure environment.Plan of Correction: As per regulations, appropriate staff will be maintained in the safe, secure environment.
There is only 1 resident on the secure unit with a diagnosis of dementia.
Standard #: 22VAC40-73-1150-B Description: Based on the investigation of a self-reported incident, the facility failed to secure windows in a manner that would prevent a resident elopement.
Evidence: According to the licensee, a resident was able to remove the plexi-glass from a window in the hallway and use a rope to exit the building and the safe, secure environment.Plan of Correction: Administrator will ensure that the secure unit is secure.
The plexi-glass which is there to provide extra protection against opening the windows will be reinforced and the unit will be checked daily for any objects that should not be there.
Standard #: 22VAC40-73-70-C Description: Based on an investigation of a self-reported incident, the facility failed to submit a written report with all required information regarding the incident within 7 days of the date of the incident,
Evidence: The facility notified the licensing office of a resident elopement on 10/17/2022. A full written report including the name of staff in charge at the time of the incident, name address and telephone numbers of witnesses was not received as of 10/24/22.Plan of Correction: A full written report of all incidents to include resident elopement will be submitted to licensing as per regulations.
