Comments:
The Piedmont Licensing office received an anonymous complaint in regards to North Roanoke Assisted Living. The complaint alleges that the facility has mold and physical plant issues, that the residents are not are not provided activities, given a heavy emphasis to use certain day support programs and are left unattended at doctor appointments. The complaint also has that staffing is low and that ALF staff have not had proper criminal back ground checks. The LI for North Roanoke Assisted Living conducted an unannounced complaint investigation at the facility on 4-25-19 from 9am until 400pm in conjunction with another LI and under the supervision of the LA. A tour of the facility physical plant was conducted and resident and staff records as well as other forms of facility documentation were reviewed. The LI also forwarded this complaint to other agencies for any concerns with their regulations. Based on observations and all evidence available for review the complaint was founded for physical plant violations. Other allegations were unfounded. Description: Based on observation of the facility physical plant, the facility failed to maintain the interior of the building. EVIDENCE: 1. The hallway light fixtures in units 2-4 contained debris and deceased bugs. 2. The fans in the hallways of units 2-4 contained dust and lint buildup. 3. There was a black substance on and around the vents in the ceiling of the dining area. 4. There were numerous stained ceiling tiles throughout the hallways of units 2-4. There was a dark brown spot, which appeared to be wet, on the hallway ceiling outside of room 200.

Plan of Correction: North Roanoke Assisted Living will comply with the Standard 22 VAC 40-73-(8)-870-A that pertains to the maintenance of the interior of the building. North Roanoke Assisted Living will continue to ensure that the interior of all buildings shall be maintained in good repair and kept clean. The cited violations have been corrected and ongoing maintenance continues to be done throughout the facility. The administrators and key staff members have been involved in informative review discussion and training as a goal to getting in and remaining in compliance with this standard. NRAL will continue to conduct rounds to check for any items identified in the standard. This plan of correction is submitted as required under State and Federal law. The submission of this Plan of Correction does not constitute an admission on the part of North Roanoke Assisted Living as to the accuracy of the surveyor?s findings or the conclusions drawn therefrom. Submission of this Plan of Correction also does not constitute an admission that the findings constitute a deficiency or that the scope and severity regarding the deficiency cited are correctly applied. Any changes to the North Roanoke Assisted Living?s policies and procedures should be considered subsequent remedial measures as that concept is employed in Rule 407 of the Federal Rules of Evidence and any corresponding state rules of civil procedure and should be inadmissible in any proceeding on that basis. North Roanoke Assisted Living submits this plan of correction with the intention that it be inadmissible by any third party in any civil or criminal action against North Roanoke Assisted Living or any employee, agent, officer, director, attorney, or shareholder of North Roanoke Assisted Living.
