Comments:
On 12/18/2019 three inspectors conducted a complaint investigation regarding hot water. Thermometer readings of hot water taps and interviews support the allegation that the facility was without hot water for approximately five days. The complaint is valid.

During the inspection and at the exit interview, the facility was given the opportunity to discuss the violations and to show that they were in compliance. Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to your licensing inspector within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain the following: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description: Based on tour of the physical plant, the facility failed to ensure that hot water taps available to residents shall be maintained within a range of 105 degrees Fahrenheit to 120 degrees Fahrenheit (F).

EVIDENCE

1. The hot water taps in the following residents? rooms did not reach a temperature of 105 F to 120 F:
room 163 registered at 80.1 F;
room 173 registered at 84.1 F;
room 176 registered at 82.1 F;
room 183 registered at 78.6 F;
room 185 registered at 61.8 F;
room 191 registered at 99.0 F;
room 202 registered at 96.0 F;
room 208 registered at 99.8 F.

2. Six residents were interviewed all expressing concerns with lack of hot water: ?Don?t have any hot water?; ?Been like this a couple weeks, especially on this side?; ?Only cold water in sink and shower?; ?Cold water in the past week?; ?Recently last 6-7 days, no hot water?; ?No hot water for four days. Hot water coming back gradually?.

Plan of Correction: North Roanoke Assisted Living will comply with the Standard 22 VAC 40-73-(8)-860-G. NRAL has rectified the hot water matter. However, the facility has made accommodations in other areas (on Units 3 and 4) of the facility for residents to utilize for bathing/showering purposes where hot water was inadequate. NRAL will continue to comply with the standard.

This plan of correction is submitted as required under State and Federal law. The submission of this Plan of Correction does not constitute an admission of the part of North Roanoke Assisted Living as to the accuracy of the surveyor's findings of the conclusions drawn therefrom. Submission of the Plan of Correction also does not constitute an admission that the findings constitute a deficiency or that the scope and severity regarding the deficiency cited are correctly applied. Any changes to the North Roanoke Assisted Living's policies and procedures should be considered subsequent remedial measures as that concept is employed in Rule 407 of the Federal Rules of Evidence and any corresponding state rules of civil procedure and should be inadmissible in any proceeding on that basis. North Roanoke Assisted Living submits this plan of correction with the intention that it be inadmissible by any third party in any civil or criminal action against North Roanoke Assisted Living or any employee, agent, officer, director, attorney, or shareholder of North Roanoke Assisted Living.

Description: Based on observation, the facility failed to ensure that the interior and exterior of all buildings were
maintained in good repair.

EVIDENCE:

1. Upon touring the physical plant, the overhead water pipe in the hallway outside of room 200 had a slow
ongoing drip. While standing in the hallway, water from the overhead pipe dropped on to Inspectors 1
and 2 who noticed a small puddle of water was beginning to pool on the floor.

Plan of Correction: North Roanoke Assisted Living will comply with the Standard 22 VAC 40-73-(8)-870-A. NRAL has repaired the sprinkler pipe leak and will continuously work to stay in compliance with the standard as it relates ensuring that the interior and exterior of all buildings are maintained in good repair.

This plan of correction is submitted as required under State and Federal law. The submission of this Plan of Correction does not constitute an admission of the part of North Roanoke Assisted Living as to the accuracy of the surveyor's findings of the conclusions drawn therefrom.
