A non-mandated complaint inspection was initiated on 8/3/2021 and concluded on 8/6/2021. A complaint was received by the department regarding allegations in the areas of resident accommodations and related provisions. The Administrator was contacted by telephone to conduct the investigation. The licensing inspector emailed the Administrator a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the allegation of non-compliance with standards or law, and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Standard #: 22VAC40-73-740-D Complaint related: Yes Description: Based on interview and resident record review, the facility failed to implement their written policy regarding procedures to be followed when a resident's clothing or other personal possessions are reported missing.
EVIDENCE:
1. On 5/30/2021 some clothing and possessions belonging to resident 1 were reported missing.
2. Page 2 of the facility's Personal Possessions Policy and Procedures states: "North Roanoke Assisted Living designated personnel will conduct an investigation and provide a written statement to the resident regarding their missing clothing and/or personal possessions in a timely manner.", and "...the results of the investigation shall be reported in writing to the resident."
3. Telephone interview with staff 1 reveals a written report with results of the investigation was not done, and the record for resident 1 shows notes were made about the missing items, however there is not copy of a report or results of the investigation in the record.Plan of Correction: Not available online.
