Comments:

The LI for Runk and Pratt at Liberty Ridge conducted an unannounced renewal study at the facility on 11/5/19 from 10am until 2pm under the supervision of the LA and noted 123 residents to be in care. Resident and staff records as well as other forms of facility documentation were reviewed and interviews were conducted with residents. A tour of the physical plant was conducted and a medication pass was observed. Please respond back to your LI with a plan of correction within 10 days of receipt of this notice. Standard #:	22VAC40-73-650-B
Description:	Based on a review of resident records, the facility failed to ensure that physician orders contained all required information. EVIDENCE: 1. The record for resident 10 has a physician order for Systane Gel Eye Drops, 1 drop into affected eye four times a day for dry eyes. The order does not identify which eye is the affected eye.
Plan of Correction:	Clarification obtained on order mentioned on resident 10. Audit performed on all orders and made clarifications where needed.

Description:	Based on observations of the facility physical plant, the facility failed to ensure that all cleaning supplies were stored in a locked area. EVIDENCE: 1. Several cleaning agents were noted in an unlocked cabinet in the kitchenette on the first floor. The door to the kitchenette was noted to also be unlocked on the day of inspection.
Plan of Correction:	Locks were installed on all the kitchen cabinets. Educated all staff that the kitchen door always must remain locked.
