Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 01/26/2021 and concluded on 01/29/2021. The administrator was contacted by telephone to initiate the inspection. The administrator reported that the current census was 156. The inspector emailed the administrator a list of items required to complete the inspection. The inspector reviewed 5 resident records, 5 staff records, recent health care oversight, recent dietitian review, recent medication/pharmacy review, staff schedule for the last two weeks and the facility's past three fire drills submitted by the facility to ensure documentation was complete. To ensure a thorough understanding of standards, the LI and the administrator had a discussion regarding standard 270. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Description:	Based on staff record review and staff interview, the facility failed to ensure that all direct care staff maintained current certification in first aid. EVIDENCE: 1. The record for staff 4, date of hire 02/18/2016, contained first aid certification dated 01/29/2018 with a recommended renewal date of 01/29/2020; however, staff 4 did not become re-certified in first aid until 01/28/2021. 2. Interview with staff 6 and 7 confirmed staff 4 did not maintain first aid certification for the period of 01/29/2020 through 01/28/2021.
Plan of Correction:	The licensee will ensure all direct care staff holds a current and up to date first aid certification as required.

Description:	Based on record review and interview, the facility, which accepts residents who are or who may be aggressive, failed to ensure that direct care staff received the required minimum training prior to being involved in the care of such residents. EVIDENECE: 1. The records for staff 1, date of hire 05/18/2020; staff 2, date of hire 02/18/2019; and staff 3, date of hire 05/29/2019, contained documentation that staff 1 had ?Aggressive and Catastrophic Reaction? training and staff 1, 2, and 3 had Relias training, ?Challenging Behaviors in Dementia Care?, prior to being involved in the care with residents who may have aggressive behaviors; however, this training was not provided by a qualified health professional and did not include demonstration and practical experience, as required. 2. Interview with staff 6 on 01/29/2021 revealed that the facility does accept and have in care residents who are or who may have aggressive behavior.
Plan of Correction:	The licensee has re-evaluated the aggressive training for new hires to include a demonstration.
