The LI for Runk and Pratt at Liberty Ridge conducted an on site complaint investigation at the facility on 02/14/2022. Resident records as well as other form of facility documentation were reviewed and interviews were conducted with staff. An exit interview was conducted with the facility Administrator to review findings. The evidence gathered during the investigation did not support the allegation of non-compliance with standards or law.
