Comments:
The LI for Runk and Pratt at Liberty Ridge conducted a complaint investigation at the facility on 04/15/2022 from 11:40anm until 1:30pm in conjunction with another LI. A tour of the facility physical plant was conducted and resident records as well as other forms of facility documentation were reviewed. Interviews were conducted with staff. An exit interview was conducted with the facility Corporate personnel on the day of inspection in which all violations were discussed and opportunities were given for the facility to provide any additional information. Standard #: 22VAC40-73-1040-A
Complaint related: Yes
Description: Based on observations made of the facility physical plant, the facility failed to have a system of security monitoring of residents with serious cognitive impairments, such as door alarms, cameras, constant staff oversight, security bracelets that are part of an alarm system, or delayed egress mechanisms on door leading to the outside.

EVIDENCE:

1. Several doors leading to the outside on the first floor of the facility, including the sliding doors in the Chapel, the exercise room and the doors in the stairwells did not have a system of security monitoring on the day of inspection. An interview with staff person 1 expressed that the facility does have a mixed population of residents residing in the facility.

Plan of Correction: The Facility will continue to use the old security monitoring system while exploring other alternative options to enhance resident security. Date To Be Corrected: 6/15/2022 The security camera system has been updated. Date to Be Corrected: 4/26/2022. Administrator/Designee will ensure a staff member is monitoring the cameras at all times.

Description: Based on observations made of the facility physical plant, the facility failed to ensure that protective devices were on windows in common areas accessible to residents with cognitive impairments to prevent the windows from being opened wide enough for a resident to crawl through.

EVIDENCE:

1. Several windows located on the first floor of the facility including windows in the chapel, exercise room, the McIntosh room and the blue room across from the McIntosh room did not have protect devices to prevent the windows from opening wide on the day of inspection. An interview with staff person 1 on the day of inspection expressed that the facility does house a mixed population of residents.

Plan of Correction: Protective Devices will be placed on all windows in the common areas of the facility accessible to the residents to prevent the windows from being opened wide enough for the resident to crawl through.
