The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 2/9/2022. The LI reviewed documentation, toured the facility, and conducted interviews relating to allegations of physical neglect and medical neglect of a resident by the facility.
The information gathered during the investigation does not support the allegation, so the complaint is determined to be ?not valid?; however, there are violations resulting from this complaint investigation.

Based on record review, the facility failed to ensure that the uniform assessment instrument (UAI) shall be updated whenever there is a change in the resident?s condition.
EVIDENCE:
1. The UAI for resident 1, dated 8/30/2021, indicated that the resident has wandering/passive behaviors less than weekly; however, facility progress notes from 9/1/2021 to 2/28/2022 indicated instances of aggressive behaviors toward staff or other residents on the following dates: 9/14/2021, 9/19/2021, 9/25/2021, 10/01/2021, 11/16/2021, 11/19/2021, 11/21/2021, 11/27/2021, 2/4/2022, 2/5/2022, 2/12/2022, and 2/21/2022.
2. The UAI for resident 2, dated 7/8/2021, indicated that the resident has wandering/passive behaviors weekly or more; however, facility progress notes from 9/1/2021 to 2/28/2022 indicated instances of aggressive behaviors toward staff or other residents on the following dates: 10/06/2021, 12/03/2021, 12/23/2021, 12/25/2021, 2/10/2022, 2/12/2022, and 2/21/2022.Plan of Correction: Facility will update UAI when there is a change in the residents? condition.

Based on record review, the facility failed to ensure that individualized service plans (ISPs) shall be reviewed and updated as needed for a significant change of a resident?s condition.
EVIDENCE:
1. The ISP for resident 1, dated 8/30/2021, indicated that the resident has wandering/passive behaviors less than weekly; however, facility progress notes from 9/1/2021 to 2/28/2022 indicated instances of aggressive behaviors toward staff or other residents on the following dates: 9/14/2021, 9/19/2021, 9/25/2021, 10/01/2021, 11/16/2021, 11/19/2021, 11/21/2021, 11/27/2021, 2/4/2022, 2/5/2022, 2/12/2022, and 2/21/2022.
2. The ISP for resident 2, dated 7/19/2021, indicated that the resident has wandering/passive behaviors greater than weekly; however, facility progress notes from 9/1/2021 to 2/28/2022 indicated instances of aggressive behaviors toward staff or other residents on the following dates: 10/06/2021, 12/03/2021, 12/23/2021, 12/25/2021, 2/10/2022, 2/12/2022, and 2/21/2022.Plan of Correction: Facility will update ISP when there is a change in condition resulting from a change in UAI.
