Comments:

An announced initial inspection was conducted on 12/16/19 by 1 licensing inspector from approximately 3:15 p.m. - 4:15 p.m. An initial inspection has been conducted due to change in ownership. There are currently 86 residents in care at the facility. During the inspection, a tour of the facility was conducted, 1 staff and 1 resident record was reviewed, measurements were taken to determine license capacity request and some policies were reviewed. All required items to be posted in the facility were observed. Recommendation made for conditional license to be issued. An exit meeting was held with the administrator. A violation was cited during this inspection. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return to the Inspector within 10 days. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description:	Based on review of staff records and facility documentation, the facility failed to ensure that all staff criminal history records are checked through the Virginia Department of State Police, Central Criminal Records Exchange. Evidence: 1) Staff #1 (hire date: 12/10/19) record contained criminal history record results obtained through a vendor company named Sterling; the report was dated 12/9/19.
Plan of Correction:	Staff #1 will have Virginia Criminal Background Check form placed in file by 12/26/19. The Business office manager will file background checks from Sterling as well as Virginia State Police. ED/designee will verify that current staff files have Virginia State Background check form by 12/31/19.
