Comments:
A non-mandated complaint inspection was initiated on 11/5/2021 and concluded on 11/29/2021. A complaint was received by the department regarding allegations in the areas of resident care and related services and building and grounds. The administrator was contacted to conduct the investigation. The licensing inspector emailed the administrator a list of documentation required to complete the investigation,
The evidence gathered during the investigation supported one allegation of non-compliance with standards or law, and a violation was issued. A violation not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Description: Based on a review of the Resident Food Order forms, three well balanced meals were not pro vided to one resident.

Evidence: An allegation was received that three meals were not being provided to resident # 1 as agreed upon in the resident agreement and individualized service plan. Resident Food Order forms for resident # 1, for October 25, 2021 through November 4, 2021, were submitted by the facility for review by the licensing inspector. The Resident Food Order forms did not document the provision of three meals a day, the forms documented two meals on 10/25, 10/27, 10/28, 10/29, 11/1 and 11/2 and one meal on 11/3 and 11/4.

Description: Based on a review of file documentation for resi dent # 1, the facility failed to document orders by a prescriber for one resident.

Evidence: An ADL(activities of daily living) order form for October 2021 for resident # 1 was reviewed during the complaint investigation. The form documented the following orders: (1) rounds every 2 hours, (2) change linens every week and as needed, (3) document attended meals, (4) hydration pass. The form contained 31 days - October 1 to October 31 2021. 1. Rounds - were not documented on the 11p-7a shift; were not documented on 10/1, 10/2, 10/20-22, 10/24-31 on the 3p-11p shift; were not documented on 10/1-10/20, 10/22, 1025, 10/26, 10/28-31 on the 7a-3p shift. 2. Hydration Pass- was not documented on the 11P-7 a shift; was not documented on 10/1, 10/2, 10/22, 10/24-31 on the 3p-11p shift; was not documented on 10/1-10/20, 10/22. 10/25, 10/26, 10/28-10/31 on the 7a-3 p shift. 3. Attended Meals-was not documented for dinner on 10/1,10/3, 10//20-22, 10/24-31; was not documented for lunch on 10/1-22, 10/25, 10/26, 1028-31; was not documented for break fast on 10/1-10/20, 10/22, 10/25, 10/26, 10/28-10/31 4. Change bed linens - was not documented (none of the days checked).
