Comments:

A monitoring inspection was conducted on 4/24/2022. A self-reported incident was received by the department regarding allegations in the areas of Additional Requirements For Facilities That Care For Adults With Serious Cognitive Impairments. The licensing inspector inspected the safe, secure environment (picture taken) and interviewed staff. The evidence gathered during the investigation supported the self-report of non-compliance with standards or law, and the violation cited is documented on the violation notice issued to the facility.

Standard #:	22VAC40-73-1180-B
Description:	Based on an inspection of the safe, secure environment, interviews with staff and a review of file documentation, an object that is harmful was accessible to a resident. Evidence: Resident # 1 resides in the safe, secure environment due to an inability to recognize danger or protect his own safety or welfare. The Department received an incident report dated 4/14/2022 that resident # 1 was "observed vomiting in the hallway and noted holding a dishwashing pod in his hand". The resident ingested and had access to dishwashing pod(s) left unattended by staff.
Plan of Correction:	ED, DON and a BTR staff will ensure all harmful items are secured and locked to prevent residents on cognitive unit from harming themselves or others. Daily and random walk through by management to check for unsecured items. BTR staff to check rooms every shift and as needed for harmful items, laundry and nurses station to be locked at all times.
