Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on March 2, 2021 and concluded on March 9, 2021. The Administrator was contacted by telephone to initiate the inspection. The Administrator reported that the current census was 47. The inspector emailed the Administrator a list of items required to complete the inspection. The inspector reviewed 3 resident records, 3 staff records, fire inspection, health inspection, resident council notes, activities calendar, staff schedules, healthcare and nutritional oversights submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-73-610-B
Description: Based on record review and discussion, the facility failed to ensure menus for meals documented substitutions or additions shall be recorded on the posted menu.

Evidence:

1. The facility?s menus failed to specify items served on the menus from 03-14-2021 to 03-27-2021 include ?beverage of choice?, ?muffin?, ?assorted cereal?, and ?choice of ice cream?

2. Staff #1 could not provide the menu with documented substitutions and additions recorded on the posted menu.

Plan of Correction: 1. The lists of various `assorted? items were added to the posted menus and the individual menus the residents receive on 3/19/2021.

2. The Food and Nutrition Services Director will complete and 100% review of the posted menus to ensure that all various `assorted? items are listed out every time.

3. The Food and Nutrition Services team members will be educated on the importance of ensuring that the various `assorted? items are listed out on all posted menus as well as the daily menus residents receive to write their orders on by 4/2/2021

4. The Food and Nutrition Services Director will audit 4 resident?s daily menus weekly for 4 weeks to ensure that all `assorted? items are listed out and clearly identified, then 4 residents per month for 4 months. Any variances will be addressed and reported to the QAPI Committee for continued analysis and improvement.

5. Corrective action will be completed by 4/9/2021.
