Comments:

An unannounced monitoring inspection was conducted on April 30, 2019. At the time of entrance seven residents were in care. The sample size consisted of four resident records and six staff records including a review of criminal background checks for new hires since the last mandated inspections. Additionally, individual interviews were conducted. Residents were observed eating breakfast and engaging in activities. Medication administration was observed. Violation notice issued with the assessed risk reviewed and exit interview held with Administrator. Description:	Based on observation, the facility failed to ensure that only oxygen from a portable source shall be used by residents when they are outside their rooms. The use of long plastic tether lines to the source of oxygen outside their rooms is not permitted.
Plan of Correction:	Administrator will discuss with family the need for portable oxygen tanks and begin using immediately.
