Comments:
A complaint investigation was conducted on 5/1/19 to address issues of resident care and accommodations. Four resident files were reviewed and a random sample of resident rooms in the memory care unit were inspected. As a result of this inspection the complaint was found to be not valid. Violations were cited but were not a part of the issues cited in the complaint.

Description: Before direct care or companion services were initiated, the facility failed to obtain, in writing, information on the type and frequency of the services to be delivered to the resident by private duty personnel. The services to be provided were not listed on the resident's individualized service plan. The facility did not ensure that the required tuberculosis screening was maintained by the facility. There was no evidence that the required orientation and training regarding the facilities policies and procedures related to the duties of private duty personnel was provided. And, it did not describe how the facility will monitor the delivery of services to the resident by the private duty personnel. Evidence: On the day of this inspection resident #1 had a private duty sitter/companion. The private duty personnel was employed by the resident's family and began employment on the day of the inspection, 5/1/2019. The facility was unable to provide the above required documentation as evidence that the requirements of this standard were met.

Plan of Correction: Random audits of current private duty personnel to be completed to insure all elements of this standard are met. Family members or other stakeholders requesting private duty personnel will meet with the Executive Director or designee to review and obtain all elements for this standard PRIOR to the start of any private duty service. Person(s) responsible: Executive Director/designee, Health Care Coordinator/designee. May 20th and ongoing.

Description: The facility failed to include the services provided by the hospice organization on the resident's individualized plan of care. Evidence: Resident #2 is receiving hospice services through Heartland Hospice. Hospice care is listed on the resident's individualized service plan and comfort care is being provided daily. However, it is unclear as to the difference between the role of the facility staff and hospice staff in the care provided to the resident.

Plan of Correction: Random audits of all ISP's of those on Hospice services to ensure the ISP reflects all services delivered by the hospice service as well as those services provided by the community. The responsibility for any services provided to the resident shall be indicated on the ISP. Random audits to be completed monthly to insure all the elements of this standard are met. Person(s) responsible: Health Care Coordinator/Designee, Executive Director/Designee. May 20, 2019 and ongoing.
